One of the most persistent myths in skincare is that acne is caused by “dirty skin” or poor hygiene. In reality, washing your face too aggressively can actually make breakouts worse by stripping your natural skin barrier.
To clear acne for good, you have to look beneath the surface. At Ziva Wellness, Dr. Vini Singh approaches acne as a complex, inflammatory condition driven by four interrelated biological factors.
Driver 1: Excess Sebum (Oil) Production

Your sebaceous glands produce an oily substance called sebum to keep your skin hydrated. However, under the influence of hormones (particularly androgens), these glands can go into overdrive. Excess oil creates a thick, sticky environment inside the pore that makes it hard for dead skin cells to shed naturally.
Driver 2: Hyperkeratinization (Cellular Clogging)

Normally, dead skin cells rise to the surface of the pore and flake away smoothly. In acne-prone skin, a process called hyperkeratinization occurs: dead cells stick together inside the lining of the hair follicle instead of shedding.
[Sticky Sebum] + [Clumped Dead Cells] ➡️ Micro-Comedone (The Initial Plug)
This sticky mixture forms a microscopic plug (a micro-comedone), laying the groundwork for blackheads, whiteheads, and deeper cysts.
Driver 3: Cutibacterium Acnes Overgrowth

Inside every pore lives a naturally occurring bacterium called Cutibacterium acnes (C. acnes). Under normal conditions, it lives harmlessly on the skin. But when a pore becomes plugged with trapped sebum, it creates an oxygen-free environment. C. acnes feeds on the trapped oil and multiplies rapidly.
Driver 4: The Inflammatory Response

As C. acnes proliferates, it secretes enzymes that break down sebum into irritating free fatty acids. Your immune system detects this damage and responds by rushing white blood cells to the area.
This inflammatory cascade turns a simple blocked pore into a red, swollen, painful papule, pustule, or deep cystic lesion.
